GeriCloud was built by a board-certified geriatrician who experienced firsthand the documentation burden and reimbursement gap in chronic care management. Practices were delivering real care every month: coordinating medications, managing chronic conditions, fielding calls between visits. And most of it was disappearing into spreadsheets or never being recorded at all.
The result is a platform built from the clinical side out: designed for the workflows care managers actually use, compliant with the programs as CMS actually defines them, and honest about what the software does and doesn't do.
